Dale Tate is the clinic's Primary Care Provider. Dale is a licensed Advanced Practice Registered Nurse (APRN), also referred to as a Nurse Practitioner (NP). Dale and his staff are Kentwood natives and they continue to live in and around the Kentwood area. Their goal is to provide daily, quality healthcare to all residents of the Kentwood area, ages 1 year and up.
“We are a rural community,” says Dale. “People in Kentwood will have choices now. We will offer quality primary care for patients in the immediate community and surrounding area on a very regular basis, as well as accept emergency walk-in patients for minor illnesses or injuries.”
The clinic opened on October 1st, 2015. It's on the campus of Kentwood Manor Skilled Nursing Facility — an award winning long-term care facility with 108 beds and a recently opened special section for short stay transitional care for those making the adjustment from hospital to home. “It’s about improving the quality of life and giving back to the community. People will have high quality healthcare without having to leave Kentwood for the first time in years," says Sheldon Dykes, Administrator of Kentwood Manor. "When you have to drop everything and drive 40 miles for primary care it can be difficult. It’s going to be a huge benefit to offer these services right here in the community.” Also, Dykes says there are future additions already being discussed to further meet healthcare needs in Kentwood. “We want to get to the point where we have different consultants — say a Cardiologist one or two days a month, a Podiatrist, different specialties,” Dykes says. In addition to the family practice clinic, Dale cares daily for the residents and patients of Kentwood Manor. He rounds each weekday morning at the Nursing Facility and is available to patients practically 24/7. "These Kentwood Manor residents have already carved a special place in my heart," explains Dale.
